Profile Test

Purpose of the test 5-HIAA test is done to measure the amount of 5-HIAA (5-hydroxy indole acetic acid) in the urine. This is a derivative product of the serotonin hormone. 5-HIAA is required by the central nervous system, mainly brain, and by cells of gastrointestinal tract and lungs. Usually, the body produces this component in very small amounts. A Large amount of 5-HIAA is produced by patients who are suffering from carcinoid tumors and neuroendocrine tumors (NETs). Carcinoid tumors can be malignant or benign and these are usually found in gastrointestinal tract, appendix, and lungs. The 5-Hydroxy indole acetic acid test is also used to detect a disorder called systemic mastocytosis (it is a condition in which the mast cells increase abnormally in size in many organs such as bone marrow). This test is very time consuming and difficult to perform. Nowadays better tests have come up with advanced technology and are easy to perform.      

Preparations of the test There are no as such preparations for the test. But your health practitioner may give you some of the basic instructions before appearing for this test. Sample required: 24 hours urine sample. The urine sample is needed to be collected before 24 hours of the appearance of the test. The instructions will be given by your health practitioner and you need to follow those instructions accordingly. Basically, the urine sample required should be normal urination; hence, there is no need to worry about it. Sample collection: on the 1st day, urinate in the morning as soon as you wake up. Then for the next 24 hours, collect the urine in a special container whenever you urinate. On the 2nd day, urinate in the morning after getting up early and then collect that urine as well.

  • For infants, clearly, wash the urethra and then place a plastic bag for urine collection. This can take some time because the infant may move the plastic bag.
  • For males, put the entire penis in the plastic bag, urinate and collect the urine.
  • For females, the plastic bag should be placed around the labia region.
  • Then, seal the container and transport it to the laboratory carefully for further diagnosis and interpretations.
It may happen that your practitioner may ask you to avoid certain food products as they can interfere with the calculation of 5-HIAA levels in urine such as plums, pineapples, bananas, eggplant, tomatoes, avocados, and walnuts. If you are taking any medicines, then you should inform your health practitioner prior to this test as some of the medicines or steroidal drugs can interfere with the diagnosis of the disease. Some medicines which can increase the quantification of 5-HIAA in urine is:
  • Acetaminophen
  • Phenacetin
  • Glyceryl guaiacolate (this component is present in most of the cough syrups)
  • Reserpine
Some medicines which can decrease the quantification of 5-HIAA in urine is:
  • Heparin
  • Methenamine
  • Phenothiazines
  • Tricyclic antidepressants
  • Isoniazid

Components of the test There are no further components of this test. The only component of this test is 5-HIAA which is measured in the urine sample.

Interpretations of the results The 5-HIAA levels in the urine sample are measured in micrograms per 24 hours (mg/24 hours). Please note that the range given in the results for your report may vary from one laboratory to another. Hence, you should confirm the test results with your respective health practitioner. >25 milligrams per 24 hours Higher values This condition indicates big carcinoid tumors especially in the Biliary region, pancreatic region and duodenal region.
Between 7-25 milligrams per 24 hours Intermediate values It indicates the possibility of disorders such as cystic fibrosis, carcinoid tumors in foregut and midgut region, Whipple disease (it is a systemic disease which can cause absorption problems in small intestine, cardiovascular system, nervous system and other bodily parts such as joints).
<2 milligrams per 24 hours Low values It can be due to depression, systemic mastocytosis, renal diseases etc.
